WELLNESS PROJECT
Alpha Resource Center, in collaboration with the Santa Barbara Neighborhood Clinics and the Hinchee Foundation, has initiated a Wellness Project to help people with developmental disabilities secure comprehensive health care services. The Wellness Project is currently accepting applications from individuals who have been unable to obtain needed medical/clinical services due to lack of funding, inadequate insurance or other means of financial support; who have medical/clinical problems or needs which require comprehensive assessment; who are unable to obtain responsive care due to the lack of coordination, poor follow-through, gaps in service or other systemic barriers; or who otherwise require specialized assistance in order to address individual health needs.
Upon receipt of a completed application, project staff will determine if the project is able to respond to the individual's needs. For approved applicants, an initial assessment will be completed, including a case history, statement of individual service outcomes and a health and wellness plan. The project staff will implement the plan and procure/provide the services needed to achieve the desired outcomes. In order to promote achievement of individual outcomes, the Wellness Project may:
- Provide direct funding to obtain clearly identified services for which no alternative source is available;
- Provide technical assistance and advocacy support to secure needed services through generic resources and programs;
- Coordinate a comprehensive wellness screening to more clearly define the nature of the individual's need or develop suitable strategies to address the need and define a responsive treatment plan;
- Provide the support of a wellness advocate to assist, monitor and guide the consumer through the treatment process, as well as to facilitate cross-disciplinary communication and coordination; and/or
- Provide or procure specialized services to ensure appropriate response to the unique health requirements of the applicant.
- In addition, a follow-up wellness plan will be developed for each participant to promote maintenance of progress, ensure continuing monitoring when needed, and secure any required follow-up support/medical/clinical services.
APPLICATION PROCEDURE
Applicants for the Wellness Project must:
- Have a developmental disability, as defined in the Lanterman Developmental Disabilities Act;
- Reside in Santa Barbara County;
- Complete the project application; and
- Participate in project evaluation.
The application consists of two elements: a Statement of Need and an Authorization and Consent to Obtain and Release Confidential Information. The Statement of Need may be completed by the applicant or by any responsible individual or organization on behalf of the applicant. The Authorization is needed to enable project staff to obtain and release information and must be completed by the appropriate legal agent for the consumer.

Completed applications will be assigned a priority based on the date of receipt. Incomplete applications will be returned to the applicant for completion and resubmission. Approved applicants will receive notification within two weeks of receipt of the application. Applicants who are not approved will be notified of the reasons for denial and advised of any steps necessary in order to qualify.
